TREATMENT OF ANIMAL GLAUCOMA



Introduction Transscleral cyclophotocoagulation (TSCPC) is a laser procedure that reduces intraocular pressure by slowing the formation of aqueous humor. The surgeon applies laser energy through the conjunctiva, fascia levator, and sclera, aiming to coagulate proteins in the ciliary body cells. After treatment, reduced aqueous humor inflow reduces the fluid load to poorly functioning drainage channels, similar to the effects of various drugs (such as beta-blockers). The patient’s response depends on the treatment method and the degree of pre-treatment aqueous humor outflow obstruction. Indications and Contraindications Indications: Eyes with failed previous filtering surgery, limited visual potential and maximally acceptable medical therapy still causing excessive intraocular pressure, painful glaucoma with highly elevated intraocular pressure and little visual potential, severe surface scarring that precludes additional filtering surgery, or eyes with the early onset of neovascular glaucoma (open-angle stage).
TSCPC may be helpful in eyes that have undergone penetrating keratoplasty, uveitic glaucoma, intravitreal silicone oil injection, refractory pediatric glaucoma, and refractory glaucoma that has failed previous tubular drainage surgery. Patients with medical contraindications or who refuse invasive surgery may be suitable for TSCPC. Contraindications: Not for completely obstructed eyes, as inflow cessation is required to reduce intraocular pressure to acceptable levels. TSCPC is generally not recommended in cases of atrophic eyes with disorganized structures or where there are other alternative and more effective treatment options, especially in eyes with good vision or at risk for macular edema.
